SIDC can assure that all written or electronic records will be accessed only by those individuals who have been given authority to do so. While recognizing these principles, the issue of privacy / confidentiality shall not be used as a barrier to the implementation of computerized information or to the rendering of care to the patient in the most effective means possible.

INFORMATIONS

Medical Information:

  • Medical and other health information when documented and collected is important for understanding the patient his or her needs and for providing care and services over time.
  • The Clinic respects all such information’s as confidential and has implemented processes that protect such information from loss or misuse
  • The information shall be released as required by DOH laws and regulations.
  • The Clinic staff respects patient confidentiality by not posting confidential information at the counters and by not holding patient-related discussions in public places.
  • The Clinic staff shall be made aware of laws and regulations governing the confidentiality of information and shall inform the patient about how the Clinic respects the confidentiality of information
  • Patients are also informed about when and under what circumstances information may be released and how their permission will be obtained.
  • The patient shall have access to their health information as per the DOH standards, patients shall be granted access to their medical record in the presence of their treating physician.
  • The following documents can be provided to patient ONLY upon their request:
  • -Medical Report
  • -Copy of investigations
  • Copies of other documents will be provided to patients upon their request after taking approval of the treating physician.
  • The Clinic maintains confidentiality of data and information and is especially careful about preserving the confidentiality of sensitive data and information.
  • The Clinic shall maintain a balance between data sharing and data confidentiality.
  • The Clinic maintains high level of confidentiality for different patient’s record and any patient clinical data and information.
  • It is the policy of the Clinic to protect all patients’ rights of privacy. However, it is recognized that the right of privacy is not absolute and must be weighed and balanced against competing situations.
  • The management will take all necessary measures to assure the maintenance of the patient’s rights with special emphasis on these rights.
  • It is the obligation of each employee, member of the medical staff and administration to protect the confidentiality of any private information which may be acquired from a patient or from any source about a patient. The trust that is built in an employee-patient relationship would be broken by disclosure of confidential information.

Other Sensitive Information:
Other information that are identified as confidential/sensitive or are known to be sensitive in nature such as financial information must be kept confidential by Clinic staff who keep that information or learns about this information as part of their job responsibilities.

Patients’ Rights at Skin Institute Dermatology Clinic:

  • Be fully aware of your rights and responsibilities and all rules governing patient care behavior and financial services / policies /responsibilities.
  • To be accepted regardless of national origin, race, age, sex, religion, disability or other factors unrelated to the provision of appropriate medical care.
  • To be treated with respect and dignity, recognition of your personal needs and sensitive to your psychological needs including the need for privacy and confidentiality in all aspects of treatment.
  • Get relevant and current information to be understood about your medication and treatment.
  • Find out who your primary doctor is and share with him in planning your care.
  • You are entitled to a full explanation from your doctor / health professional about your medical condition and the need for recommended treatment / appointment, including risks, side effects and other treatment options / appointments before approval of treatment / appointments.
  • You should be aware of participation in all aspects of your individual care plan and be aware of the right to refuse treatment and be fully aware of the medical consequences of the refusal of treatment.
  • You may review your medical record by the Director of the Clinic or his representative and a time agreed between you and the Director of the Clinic or his representative in advance.
  • You are entitled to request a copy of your medical records.
  • Get the necessary services as outlined in your individual care plan.
  • Getting your medical records and information received will be considered private and confidential.
  • You may freely express grievances verbally or in writing to the Clinic staff, management,organization of the Human Rights and Development Network and appropriate regulatory bodies without fear of reprisal or discrimination.
  • Information about the grievance process will be provided to you and the Clinic Patient Care Coordinator will assist you if necessary.

Patient Responsibilities at Skin Institute Dermatology Clinic:

  • The patient is responsible for compliance with the financial policies of the entity and requests the signing of the waiver of interest forms, which includes approval of the publication of medical information to the insurance companies.
  • The patient shall be responsible for providing the Clinic with correct and accurate information of identity, population and insurance coverage when registering with the clinic and throughout the period of receiving treatment from the clinic.
  • It is your responsibility to cooperate with the Patient Care Coordinator and keep him informed of changes in your name, address, insurance, financial status, medical guidance, changes in your family /support system and other changes that may affect your health care. And your ability to meet your financial obligations for the services provided by the clinic.
  • Attend the treatment / appointment as scheduled and arrive on time. If you want to change your route in anyway.
  • You have a responsibility to abide by all aspects of treatment as prescribed by your doctor. If you do not agree with the prescription or want to make changes, you should discuss them with your doctor. Your doctor is the only person who can request these changes.
  • You are responsible for treating other patients, visitors and Clinic staff with respect.
  • You are requested not to disclose the information of other patients’ cases and treat them as confidential is an important part of your health care team and you are encouraged to participate in the planning process.
  • You have the responsibility to tell employees if you experience any health problems between the treatments, your doctor, new medicines and / or your old medicines have been changed.
  • During the period between treatments, you are responsible for following the directions of the health care team designed for you to avoid health problems.
  • You are responsible for following the infection control and safety policies of the Clinic.
